The first stage of your application is to complete our mystery shopper application form.
To be considered for any work as a mystery shopper with the Therapy Agency, you need to have completed and sent back our application form.

If you have difficulties accessing the application form, and have tried updating your free version of Acrobat Reader with no success, please contact us, so we may send it to you by an alternative method (such as by fax, as an email attachment, or by postal mail).
If you are unable to complete our application because of a disability, please contact us with details so we may provide you with an application in a suitable format.
When returning your application, please ensure that you have enough postage on your application (normally three 2nd class stamps are sufficient for weights up to 250g which is a typical application, but please check the Royal Mail website or your local post office to be sure). As detailed on the form, you will need to send your completed application to:
